A near miss is a situation that could have caused damage, pollution, ill health or injury. On this occasion, it hasn’t – but it still needs to be reported. A near miss must be reported immediately.

Here are some examples of near misses:

Not wiping surfaces thoroughly which could have led to ill health An oil spill on deck, cleared before it enters the sea A person slips on spilled water, but does not fall A Guest is found in a crew area, on this occasion no harm occurred
